Each resident's needs and preferences are assessed prior to admission and are reassessed quarterly or as the resident's needs change. A detailed care plan is developed and maintained for each resident based on the assessment information. Through a number scale, we categorize each resident's needs into one of five levels. This allows us to coordinate daily care as well as determine the appropriate monthly rate.
Levels of Care
A typical Level I resident directs easily, is continent of bowel and bladder, has no significant other medical conditions, and is independent in most activities of daily living (ADLs).
A Level 2 resident requires more cueing with ADLs, may occasionally be incontinent, and may have other medical conditions.
At Level 3, the resident requires much cueing or redirection, needs some help with most ADLs, is regularly incontinent, and may have other medical conditions.
At Level 4, the resident requires full assist with ADLs, near constant cueing, near constant incontinence management, and may have other significant medical conditions.
At Level 5, the resident requires full assist with ADLs, requires full incontinence management, one-on-one care, and may have other significant medical conditions and or behaviors.
Dungeness Courte provides nursing services 24 hours a day with full time supervision of a licensed nurse (Resident Service Director). We provide nursing services within the scope of the Washington Administrative Code chapter 388-78A. Our services include, but are not limited to: medication administration; routine health assessment; monitoring of residents health condition; diabetic monitoring; wound care; and working with other health care professionals to provide the optimum health care for each resident.
