Admission Process

If you need help with your loved one, The Courtyard can respond 24 hours a day. Our staff can conduct an assessment to determine appropriate care plans and can admit a resident anytime, day or night.

The admission process can be very overwhelming and very stressful for families. Our admission coordinator can alleviate the stress by guiding and assisting the families and familiarizing them with the procedures.

Families receive a form of what necessary information is needed from the beginning, "What to Bring." They will also be issued a Resident Handbook, Legal Rights Booklet, and Glossary of Terms and Resident Rights Booklet.

A POLST form is filled out by the family and signed by the attending physician. Physician orders along with a History and Physical are submitted for the physician to fill out and return to the facility. She orientates the families with the facility handbook while gathering the necessary paperwork needed to complete the admission process.
