Each resident’s needs and preferences are assessed prior to admission and are reassessed quarterly or as the resident’s needs change. A detailed care plan is developed and maintained for each resident based on the assessment information.

The Courtyard will carefully assess the resident’s individual needs to ascertain the appropriate care needed. The Courtyard assessment team is dedicated to ensuring that a comprehensive care plan is developed that will emphasize the resident’s functional strengths. A pre-admission nursing assessment is required for all Day Care, Respite Care, and Long-term Care residents.

Basic Services include meals and snacks, weekly housekeeping and maintenance, assistance with medication, Life Enrichment programs, and cueing for activities of daily living.

In addition to the basic services provided to every resident, The Courtyard offers additional services and care for residents who need extra personal care and attention.
